name) & Sex
The Name should be exactly the same as your passport.* Airlines may refuse
boarding if your name is not the same as passport, or you may be required
an additional charges to change the name.
2 )Phone Number & Fax Number or your e-mail adress
3 )Nationality
Also please tell us if you already have your Re-entry permit.*
4 )Permanent Address in Japan

We will send you our application form after booking. Please submit our
application form right after you received it with copy of your passport.
*About your name on Passport
Some airlines require even middle name of your passport, so that we recommend
you to tell us your full name exactly
the same as your passport.
Also we will appreciate if you send us copy of your passport in order to check the spell.
We are not responsible if you cannot enter some contries because of the
rules concerning passport validity.
Please note that the passport validity depending on your nationality and
the country you are going to.
*Re-entry Permit
If you are not Japanese citizen, you may have to get a Re-entry Permit at the nearest immigration office before you leave Japan. You will lose your working, resident, and permanent visas If you leave this country without it.
JST is not responsible for refusal of entry into or departure from any
country because of lack of above documents.
